JOB DETAILS:
Job summary:
Ensure a patient-centered care in all programs of Operation Smile in Rwanda. Coordinate the implementation of a national registry and database of cleft lip and/or cleft palate patients throughout the country. Work closely with various community leaders and partners to set up a national registration campaign. Work closely with the Rwanda Program Coordinator to ensure all cleft and other patients are located, properly screened and, if the patient receives surgery, follow up care is provided either during a mission or by a referral surgeon. Ensure barriers to care are reduced during Operation Smile programs, mainly the availability of patient accommodation, transport, meals and nutrition.

General roles and responsibilities
Patient Recruitment/Referrals, Patient Advocacy program, Awareness and National Registry
• Work closely with the Ministry of Health and its Peripheral hospitals to arrange meetings with authorities in the areas; find focal persons at each health care facility or in each town/District and establish a working relationship with them through awareness campaigns. Use the existing system in place to reach out to patients and especially all local health structures and community agents to register cleft patients from birth.
• Access existing community health care worker structures, set up trainings on cleft recognition for health care workers and arrange to implement cleft awareness programs in the community.
• Collate patient data forwarded from community health care workers, chiefs, and patients/guardians into regional and national data bases.
• Develop regional partnership through churches, schools, NGO or associations, and corporate sponsors to set up partnerships focused on awareness, patient recruitment and/or sponsorships; strengthen relationships with organizations referring patients to us.
• In charge of Patient Advocacy program (following up with PAs, Identify PAs to recruit)
• Manage Hotline number and other patient recruitment channels (Facebook messages,)
Detailed responsibilities
• Primary responsible for the Patients (find an appropriate District, work closely with the district team members, in charge of patient medicines, in Charge of Pre-screening at the District Hospital.
• Arrange patient supplies for distribution on arrival at the District.
• Coordinate and schedule patient workshop itinerary with partners and volunteers at the Village.
• Primary responsible for patient national transport (from home to mission site and back)
• Primary responsible for patients’ expense reimbursement.
• Managing Nutrition Program, its database and follow up.
• Create a database, database for patient referrals.
• Create patient return lists after patient announcement; ensure all patients have transportation home.
• Create and manage a database of all the contacts that Operation Smile has in each province/Regions such as, local NGO, Church leaders,
• Ensure that all patients will be uploaded in our database.
• Ensure that all patients will be contacted leading up to the mission to confirm their arrival and approximate number expected for screening.
• Creating a database for all Patient transport cost for in-house reference (National, Regional.
Other duties
• Support Program Coordinators with mission logistics and education programs.
• Work closely with the Program Manager and Program Coordinator to identify and implement appropriate care needed by each patient before and after surgery: nutrition, dental, speech, psychological support.
• Create a strong relationship with patients and guardians; assess if their needs are being met.
• Organize patient information session for all patients who arrived for screening about what will happen during the mission (ex. screening process, selection and announcement, surgery days).
• Ensure barriers to care are reduced in all Operation Smile programs in Rwanda (surgical missions, dental mission, feeding program, speech) including patient supplies, patient meals, patient transport, and patient accommodation.
