Team Leader Community Clinics
• Job Overview
We have a very exciting opportunity within our newly established service!

Carmarthenshire Community Nursing services are about to launch community clinics – come and be a part of how we deliver community nursing care in a different way.

We are looking for a dynamic, motivated, and energetic Team Leader to lead and manage this new service. Clinics will be based across Carmarthenshire, and we anticipate there to be three clinics covering locations across the county, of which the successful candidate will manage all.

The clinics will develop in differing phases of clinical interventions. The initial focus will be around ear wax and ear health management. Further interventions planned will be around continence, wound care, and further interventions for non-housebound patients. As the Team Lead, you will be instrumental in the development of these clinics and the development of your reporting teams.


Main duties of the job
• This service will complement our existing district nursing home-based teams to ensure effective delivery of nursing services to patients across all localities.

• We will have a blended mix of registered, non-registered and administration staff within the teams and all clinical training including competency sign off will be provided.

• The successful applicant will have full support from Clinical Lead Nurses, and a full orientation and induction plan will be put into place.

• We would welcome applications for nursing staff who are or have worked at a senior level in the community or clinic environment with substantive experience.

• The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
• Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
• Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
• 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
• Numerous locations providing mental health and learning disabilities services;
• Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
